Every Field Trip Form needs that AHC number!
Each year our Crusaders take road trips - it could be a class trip to Blue Quills or a sport event like track and field. On these trips, supervisors must take the permission forms that you fill out. In the unlikely event that a medical emergency occurs, your child's Alberta Health Care number must be shared with the receiving hospital. "On File" can not be used in this form field. The actual number is a MUST!
Recently, we have been receiving field trip forms without the AHC number listed. Please fill this in as it is not possible for the secretarial staff to search for 50 AHC before a trip.
Here's a helpful hint: Give your child their Alberta Health Care number and ask them to put it into the notes app on their phone - and put it in your phone! Then the number is a quick click away!
