    Each year our Crusaders take road trips - it could be a class trip to Blue Quills or a sport event like track and field. On these trips, supervisors must take the permission forms that you fill out. In the unlikely event that a medical emergency occurs, your child's Alberta Health Care number must be shared with the receiving hospital. "On File" can not be used in this form field.
